Your visit may include:
- Review of your medical history and vaccination records
- Physical exam by our USCIS-certified Civil Surgeon
- Verification and documentation of all vaccinations required by USCIS
- Ordering of required lab tests
- Completion of Form I-693, and submission to eMedical
What to Bring to Your Appointment
To help your appointment go smoothly, please bring:
- Original Passport
- Digital or physical Vaccination Records in English or other languages (if any)
- USCIS A-Number (if any)
- Annual physical exam lab report from the past year (if any)
- Previous TB test or chest X-ray records (if any)
- Previous I-693 form (if any)

Simple 3-Step Process
Step 1 — Schedule Your Appointment
Book online by clicking “New Patient” and completing the registration forms.
If you have PPO insurance for vaccines or lab work, please upload your insurance card during registration. You may also call or text our office for additional appointment availability. We also offer same-day appointments and family appointment scheduling.
Step 2 — Complete Your Exam
Meet with Dr. Li for your immigration physical exam. We will order the required antibody blood tests and other labs.
A Labcorp is located in the same building and accepts walk-ins. Their business hours are: 7:00 AM–12:00 PM and 12:00 PM–3:30 PM. You may also use any other Labcorp or Quest locations; hours vary by location.
Step 3 — Pick Up Your Sealed I-693 Form
Lab results typically take 3–5 business days, so the entire processing time is generally 3–5 business days.
If you are not immune according to your lab results, you can receive the required vaccines at our clinic when you return to pick up your completed Form I-693.
Once we receive your lab results and all other requirements have been completed, you can stop by the clinic during business hours to pick up your sealed Form I-693 envelope for submission to USCIS.
