What’s Included in Your I-693 Exam
Your visit may include:
- Review of your medical history and vaccination records
- Physical exam by our USCIS-certified Civil Surgeon
- Ordering of required lab tests, including but not limited to TB, MMR, and polio screening
- Verification and documentation of all vaccinations required by USCIS
- Completion of Form I-693
If you need vaccines or additional testing, we can help arrange everything during your visit.
Once all required labs and vaccines are completed, you will receive your sealed I-693 envelope ready for USCIS submission.
What to Bring to Your Appointment
To help your appointment go smoothly, please bring:
- Original passport
- Any vaccination records you have
- Previous TB test or chest X-ray records (if available)
- Previous I-693 form (if applicable)
If you do not have vaccine records, no worries, we can still help.
Simple 3-Step Process
Step 1 — Schedule Your Appointment
Book online by clicking “New Patient” and completing the registration forms.
If you have PPO insurance for vaccines or lab work, please upload your insurance card during registration.
You may also call or text our office for additional appointment availability. We also offer same-day appointments and family appointment scheduling.
Step 2 — Complete Your Exam
Meet with Dr. Li for your immigration physical exam and required testing.
We provide all required vaccines in our clinic.
If needed, we can also order blood antibody testing. There is a Labcorp location in the same building that accepts walk-ins for lab work.
Step 3 — Pick Up Your Sealed I-693 Form
Once all requirements are completed, you can stop by the clinic during business hours to sign and pick up your sealed USCIS envelope.
